Free Application for Federal Student Aid

2013-2014 Free Application for Federal Student Aid (FAFSA)

This form is to be used by a student applying for financial aid for the 2013-2014 academic year. The school year runs from July 1, 2013 to June 30, 2014.

 

Priority deadline for financial aid for the 2013-2014 school year is March 31, 2013.

2012-2013 Free Application for Federal Student Aid (FAFSA)

This form is to be used by a student applying for financial aid for the 2012-2013 academic year. The school year runs from July 1, 2012 to June 30, 2013.

This form is to be used by the student to request that their financial aid award package be re-packaged so that they can make changes to their initial award decision. Additional documentation may be required.

Consortium Agreement Form

This form is to be used by a student to enter into a consortium or contractual agreement that allows the student to continue to receive federal student aid funds while studying at an institution other than their "home" institution.

This form is to be used by a student to verify that they are/were an emancipated minor or in a legal guardianship as indicated on their FAFSA application.

This form is to be used by a student who would like their progress re-evaluated for the purpose of reestablishing their financial aid (post-suspension).

Satisfactory Academic Progress (SAP) Waiver Request

This form is to be used (with supporting documentation) by a student who has experienced an extenuating circumstance during the academic year that led to their inability to maintain satisfactory academic progress to request that SAP requirements be waived.

Texas Grant Statement of Eligibility

This form is to be used by a student who was awarded a Texas Grant to indicate if they have ever been convicted of a felony or an offense under Chapter 481, Health and Safety Code (Texas Controlled Substances Act) or under the law of another jurisdiction involving a controlled substance as defined by Chapter 481, Health and Safety Code.

Unusual Enrollment History Appeal Form

Students may use this form to request a re-evaluation of the termination of their financial aid. Submit your completed appeal request and supporting documentation verifying extenuating circumstance to the Office of Student Financial Assistance for review by the Financial Aid Appeals Committee.
